Registration
The following information will be needed from NEW/TRANSFER student applicants:
Copy of Birth Certificate
Copy of Baptismal Certificate (if Catholic)
Copy of Social Security Card
State of Louisiana Health Card
Check for Registration ($165 for 1st student; $150 for each additional student)
Voucher, if a registered, active, supporting parishioner of a parish OTHER than SMM parish
In addition, students applying for grades 1-8 must complete the following:
Copy of Current Report Card (Transfer Students)
Copy of Standardized Test Scores (Transfer Students)
Letter Stating Reasons for Transfer (Transfer Students)
Interview to be scheduled at a Later Date (Transfer Students)
The following information will be needed from returning SMM student applicants:
Check for Registration ($165 for 1st student; $150 for each additional student)
Voucher, if a registered, active, supporting parishioner of a parish OTHER than SMM parish
